Please use the Changes Outpatient Program entry and arrive 15 minutes early for check-in.
Visitors must sign in at the reception area and complete COVID-19 screening requirements prior to seeing patients. Items brought for patients will be checked by staff, and the policy for unsafe items will be strictly followed. All visitors must lock their personal belongings in the lockers located in the lobby or leave those items in their vehicle. Additionally, they will be wanded to ensure they have no potentially dangerous item on their person when visiting.
All visitors must be able to present the patient ID number and be listed on the telephone/visitor authorization before being able to visit.
All visitors will be given a visitor badge/sticker to identify they have been checked in, and the badge must remain visible throughout the visit.
Visitors will be escorted to and from visitation areas with Hospital staff.
Visitors under the age of 13 are not allowed for on-site visitation at this time.
Visitors are requested to limit group size to two (2) visitors at one time. In the event that a visitor is suspected of passing drugs or contraband, the visitor will be asked to leave the premises immediately.
